This document is a Financial Resources Questionnaire provided by OPM to participants who have been overpaid and who seek to reduce the amount that is recouped from their monthly benefit payments because the current recoupment amount causes a financial hardship. It asks retirees to summarize their monthly income and expenses in order to determine how much the retiree can afford to pay toward the recoupment each month. This form could potentially be a helpful tool in any recoupment case because it can be used as a checklist of factors upon which a client can base a hardship waiver argument.
